Turn in Applications to:

If you have not applied online turn into your School Office or the Child Nutrition Department at the District Office (in the Student Services Building). We recommend turning it in to the Child Nutrition Department (this cuts out the time spent in transit).  We have ten days from the time it reaches the CNP Department to process the application. Keep in mind that an online application will, in general, process more quickly. Plan accordingly to insure your student's meals are paid in advance or brought from home pending processing. Any allowed long-term charges may be turned over to a collection agency.

   Federal Eligibility Application for Free or Reduced Meals

We are pleased to make available the ability to apply for Free or Reduced Price Meals online! The process is SAFE, SECURE, PRIVATE, and AVAILABLE anytime, anywhere!

You may complete your child(ren)’s application for the free or reduced price meals online as above, or the link may also be found on our district homepage www.provo.edu , or go directly to www.schoollunchapp.com.  You may easily complete the application online by answering the interactive questions.

Printable confirmation is available, after the application has been processed, as instructed online.  Meal applications will still be downloadable from our website (July through May).  Paper forms are also available (July-May) at all school main offices and the district office. (Some students will be renewed for free meals in the summer per welfare services records, with approval notification provided via mail, per PowerSchool settings for home address.)

 Please remember to only submit ONE application for your entire household.  (Foster students may be applied for separately or as part of your household.)

 If you print/fill out an internet version of the meal application, you MUST return the pages headed "FREE AND REDUCED PRICE SCHOOL MEALS FAMILY APPLICATION"

If your child received meal assistance through our program last year, you must still complete a new application each school year.  This coming year’s deadline for your re-application is September 30th (submit by September 18th to insure your child/children's benefits do not lapse).  However, please be advised that Free or Reduced applications for FY16-17 will not be available nor any version processed until mid-July.

Do not complete a new school year application if you received summer notice from Provo School District stating approval of (ALL) your child(ren) per our import of welfare services records for the stated coming school year.  [See “Direct Certification” information below.]

Keep in mind we will process your application as quickly as possible (and online applications should move quicker through the system than paper applications). During the annual renewal period of high activity this may, however, take the federally allowed ten operational days. Breakfast costs $1.50; lunch costs $2.00 Elementary/$2.25 Secondary.  When qualified, your children may be approved for free meals or for reduced price meals. Reduced price is $0.30 for breakfast and $0.40 for lunch.

Factors considered in the application process are household size and total household income. HOUSEHOLD SIZE is considered all persons, related or unrelated, who live in your home, including foster children*. The TOTAL HOUSEHOLD INCOME is the income each household member received in the last month, before deductions - this includes wages, social security, pension, unemployment, child support, alimony, and any other cash income.  If the income received in the last month is “unusual,” enter an “average” amount (also see FAQS below and the application itself for more details). Please note that income applications cannot just reflect a yearly amount. If you actually receive your income annually, so state; otherwise, we will not be able to process it until we have obtained the proper breakdown indicating how often income is actually received.

*FOSTER CHILDREN may be included in your household application or applied for separately.

CASE NUMBER - This is not an “income” application. We require your SNAP (food stamp) or FEP case number. Medicaid and Horizon numbers are not acceptable nor is Medicaid alone a qualifier for free meals.

OTHER LANGUAGES - Other language applications may be found at http://www.fns.usda.gov/cnd/Application/familyfriendlyapps.html  and used as a guideline in filling out an English form.  (If downloaded, you will encounter formatting problems which must be corrected [note above paragraph relative to the pages that must be returned].)

 

FAXED APPLICATIONS - We do accept faxed applications, but prefer hard copies.  Faxed applications do not get processed sooner than hard copies, and are unusable if not completely readable or sent in the proper format.  We do not accept emailed applications, but we do also accept online meal applications, as above.

 

DIRECT CERTIFICATION - Under direct certification, school districts use information from state welfare or food stamp offices to certify children to receive free meals. Notice goes out to households each summer prior to the start of school confirming each student's eligibility.

 

If you receive a direct certification letter for some but not all students in the household, to qualify the remaining students please fill out a free and reduced meal application. Remember to include ALL children in Part 1 (and again in Part 4 for any who have income).

 

INCOME APPLICATION COMPLETION TIPS

Apply online to expedite approval.

Use BLACK or BLUE ink if handwriting your application, and Print Clearly.

Fill out completely - many sections are required for processing.

List your student names as they appear on their Birth Certificates (step one).

List all Household members (step one and three), including students.

Fill out all applicable income columns showing amounts AND how often each amount is received.

Fill in Social Security Number for the signer or write “NONE” (step three).

Your application will be returned to you if it is not signed.
